The purpose of this website and its questionnaires or surveys (collectively, this “Service”) is to provide information and assist with decision-making only. This Service does not replace professional medical advice, diagnosis, or treatment. Available features for Users:

Personalized shared decision-making questionnaires Ability to save and share results with healthcare providers Access to their own questionnaire records if their responses were either temporarily saved or completed in full

Users must comply with all applicable laws and regulations when using this Service. Users shall not engage in any activities that are illegal or disruptive to the system. The Service Provider shall have no liability whatsoever for Users’ use of this Service and any consequences therefrom.

Membership is available only to authorized research personnel affiliated with participating institutions (hospitals). Members must provide accurate information and keep their account secure. Members can access past patient questionnaires that have been fully completed and for which the patient has provided consent. Members can view past questionnaire data from patients at their own institution (hospital) with fully completed questionnaires. Access to questionnaire data from other institutions (hospitals) is restricted. Members may withdraw membership at any time and violating these Terms of Use and/or Membership Agreement may lead to suspension or termination of membership.
